A Heart Health Value-Based Care Plan
Prevention of heart disease is a key factor to heart health, knowing the signs and symptoms is important in order to get ahead of worsening heart disease. Symptoms include numbness, shortness of breath, chest pain, chest pressure or chest tightness, lightheadedness, neck or jaw pain, sudden weakness, slow or weakening pulse, and cold sweats or coldness in your hands, arms, or legs. Patients that are predisposed to high blood pressure, high cholesterol, and diabetes get ahead of these risk factors as well as any other risk factors that may cause heart disease.
A custom care plan can address identify and address these issues early on to prevent heart disease.
How do we know?
CCM Improves Outcomes & Reduces Costs
The Centers for Medicare & Medicaid Services (CMS) released a retrospective claims analysis of two years worth of data that included every patient in the United States enrolled in a Chronic Care Management program. The data is compelling.
The Electronic Health Record (EHR) is a crucial tool for pulling eligible CCM patient lists, pushing out patient comprehensive care plans, and billing each month. ChartSpan has experience with every major EHR in the industry, and ChartSpan works with practices to establish data feeds, such as an HL7 and API.
"I believe we are delivering better quality care to patients. Our patient satisfaction is up, and our patients like that they can get in touch with someone any time, day or night."

The Top Three Preventable Heart Diseases
A customized care plan can address the most common preventable heart diseases.
Ischemic heart disease: This heart disease can also be referred to as coronary heart disease and it is when the heart is not being supplied adequate blood for its proper functioning. It can be prevented through eating a healthy balanced diet, giving up smoking, exercising more often, reducing your alcohol intake, and making sure that your blood pressure is managed.
Congestive heart failure: Sometimes known as heart failure, occurs when your heart muscle doesn't pump blood as well as it should. Certain conditions, such as narrowed arteries in your heart (coronary artery disease) or high blood pressure, gradually leave your heart too weak or stiff to fill and pump efficiently.
Hypertensive disease: This is a result of uncontrolled hypertension (high blood pressure) causing the heart to work under increased pressure which causes the heart to work much harder. In order to prevent this type of heart disease take all medications as directed and ensure that you are following a low salt diet as well as exercising regularly in order to prevent the increase of your blood pressure which can progress to hypertensive heart disease.
Last year, ChartSpan's managed CCM programs decreased overall annual healthcare costs by more than $40M, generated $29M in Fee-for-Service reimbursements and created millions of dollars in shared savings. To date, we have provided more than 1M monthly patient CCM encounters, totaling more than 20M in reimbursable minutes. Their average NPS™ patient satisfaction score of 74.2 is the highest in the industry.
We believe that proactive patient engagement is the key to achieving better outcomes. However, asking overburdened physicians and their staff to execute a care management and coordination program is impractical. ChartSpan's solutions relieve the workload while delivering valuable support, increased revenues, and better patient outcomes.
