ChartSpan, ES4P, and Johns Hopkins School of Medicine announce offering the first-ever Chronic Care Management CME Course

(Greenville, South Carolina) - October 30, 2019 - ChartSpan, the largest managed provider of chronic care management (CCM) services, has worked alongside Essential Seminars for Physicians, LLC (ES4P) to create the curriculum for the first-ever Chronic Care Management Continuing Medical Education (CME) course. The course was reviewed and accredited by the Johns Hopkins University School of Medicine and will be available for all providers nationally on November 1, 2019.

In the age of value-based care, providers are feeling the pressure to produce better outcomes at lower costs. The greatest cost to the healthcare system is servicing the elderly with chronic conditions. Eighty-five percent of older adults have at least one chronic condition and sixty percent have multiple. Healthcare providers are expected to identify and manage these patients to produce the best outcomes possible. This is best done through a scaled CCM program.

Since the CCM reimbursement program is fairly new, providers and vendors alike are still navigating the complexities of a successful CCM program. That’s why ChartSpan was driven to find a way to educate providers on the power and importance of chronic care management.

The course, titled “Medicare Chronic Care Management: Discovering the Value,” will be accessible online through a virtual classroom. Upon completion of the course, providers will have acquired knowledge of the Medicare rules of CCM, as well as a framework on how to provide the care as a profitable service line.
