ChartSpan partners with HAPevolve to bring value-based care to Medicare patients

Greenville, South Carolina – May 1, 2025 – ChartSpan, a value-based care organization, has partnered with HAPevolve, a subsidiary of The Hospital and Healthsystem Association of Pennsylvania (HAP), to offer preventative care programs to Medicare beneficiaries. 

ChartSpan, the largest full-service Chronic Care Management (CCM) organization in the U.S., is thrilled to partner with HAPevolve to offer value-based care to more Medicare patients across Pennsylvania. In addition to turnkey CCM coordination and compliance, ChartSpan provides Annual Wellness Visit software, Advanced Primary Care Management services, and quality improvement programs to 175+ health systems and tens of thousands of Medicare beneficiaries. They focus on helping Medicare patients achieve better health outcomes while lowering their healthcare costs.

With Chronic Care Management services, Medicare patients with multiple chronic conditions can benefit from monthly check-ins, personalized health goals, and a 24-hour care line. Advanced Primary Care Management offers all Medicare patients a dedicated care manager, support after a discharge from the hospital or ER, and care goals and a care line similar to those offered by CCM. ChartSpan’s Annual Wellness Visit software also makes it easy for Medicare patients to complete Health Risk Assessments before meeting with their providers.
