ChartSpan Announces Partnership with Health Resource Services

ChartSpan, the leading provider of managed Chronic Care Management (CCM) in the United States, has announced a new partnership with Health Resource Services (HRS).

HRS, a health care group purchasing organization, offers access to products, services, information, and solutions that help member providers improve operational and financial performance. Through a partnership with ChartSpan, HRS members will have access to high-quality value-based care solutions for Chronic Care Management and Annual Wellness Visits. Both organizations share a commitment to helping providers and their practices improve their bottom line.
