New Mexico Rural Hospital Network Partners with Largest Chronic Care Management Provider, ChartSpan

(Greenville, South Carolina) - October 3, 2019 - New Mexico Rural Hospital Network (NMRHN), a statewide organization among independent, rural, nonprofit hospitals in New Mexico, has announced they have selected ChartSpan as their exclusive Chronic Care Management partner.

ChartSpan is the largest healthcare managed service provider of chronic care management (CCM) programs in the U.S. The company offers healthcare providers a turn-key service for chronic care management, allowing both patients and providers to see higher levels of engagement, better adherence to prescribed care paths, and better patient outcomes. In addition, practices utilizing ChartSpan’s CCM services receive a monthly, per patient revenue stream through CMS reimbursements.
