ChartSpan partners with HAPevolve to bring value-based care to Medicare patients

(Greenville, South Carolina) – May 22, 2023 – ChartSpan, a value-based care organization, has partnered with HAPevolve, a subsidiary of The Hospital and Healthsystem Association of Pennsylvania (HAP), to offer preventative care programs to Medicare beneficiaries. 

ChartSpan currently provides Chronic Care Management (CCM) services, Annual Wellness Visit (AWV) software, and other preventative care solutions to more than 140 practices and health systems across the U.S. ChartSpan’s programs focus on helping Medicare patients achieve better health outcomes while lowering their healthcare costs.

With Chronic Care Management services, Medicare patients with multiple chronic conditions can benefit from monthly check-ins, personalized health goals, and a 24-hour nurse line. ChartSpan’s Annual Wellness Visit software also makes it easy for Medicare patients to complete Health Risk Assessments before meeting with their providers.

Through its industry partner program, HAPevolve connects Pennsylvania hospitals with services and vendors that can help improve their operations.
