ChartSpan Announces Partnership with the Community Health Center Association of Mississippi (CHCAMS)

(Greenville, South Carolina) – April 21, 2020 – ChartSpan, the largest, managed chronic care management vendor in the U.S., has announced a partnership with the Community Health Center Association of Mississippi (CHCAMS), a nonprofit membership organization of twenty-one Community Health Centers (CHCs) throughout Mississippi.

CHCAMS’ centers provide healthcare for more than 308,000 underserved patients in Mississippi. ChartSpan’s chronic care management (CCM) services provide an extra level of care to patients in-between regularly scheduled appointments, which is especially helpful to practices in more rural areas who struggle with resources and funding. A partnership between ChartSpan and an organization like CHCAMS provides the opportunity to extend preventive patient care to rural and urban areas in need of additional healthcare.
