(Greenville, South Carolina) – January 13, 2021 – ChartSpan, the largest provider of fully-managed Chronic Care Management (CCM) services in the U.S, announced a new partnership with Remote Care Partners (RCP), a cloud-based Remote Patient Monitoring (RPM) platform.

ChartSpan and RCP share a similar mission in using their offerings to strengthen the patient-provider relationship. By providing an additional source of care outside of the office, CCM and RPM are able to close gaps in care for patients who need special attention. Both companies have designed their product/service to remove barriers to care and make the offering straight-forward and easy for providers as well as patients.

After looking at the ChartSpan’s background, we realized their goals and philosophies were closely aligned with ours. We look forward to a fruitful business relationship with ChartSpan during this changing healthcare environment.” – Curt Pace, Senior Vice President of Remote Care Partners

“We look forward to working with a company that shares our mission to improve the quality of care for patients across the nation. We believe that through different methods of virtual patient support we can help to close important gaps in care. Our companies are positioned to identify unique and aligned opportunities where providers are in search of our solutions that provide a more holistic view of a patients’ health.” – Jon-Michial Carter, CEO of ChartSpan
