
(Greenville, South Carolina) – January 13, 2021 – ChartSpan, the largest provider of fully-managed Chronic Care Management (CCM) services in the U.S, announced a new partnership with Remote Care Partners (RCP), a cloud-based Remote Patient Monitoring (RPM) platform.
ChartSpan and RCP share a similar mission in using their offerings to strengthen the patient-provider relationship. By providing an additional source of care outside of the office, CCM and RPM are able to close gaps in care for patients who need special attention. Both companies have designed their product/service to remove barriers to care and make the offering straight-forward and easy for providers as well as patients.
