(Greenville, South Carolina) – April 26 – ChartSpan, the leading provider of managed chronic care management (CCM) in the U.S., announced a new partnership with the Iowa Hospital Association (IHA) and its subsidiary, ServiShare.
Through this new partnership, IHA and ChartSpan will work closely to provide turnkey, value-based chronic care management and reimbursement programs and annual wellness visit services to Iowa health care providers and hospitals. These programs improve patient outcomes and drive significant streams of value-based revenue. Leveraging a chronic care management program can improve patient engagement and health outcomes while providing a predictable source of reimbursement for practices, hospitals and health systems.
“With the Centers for Medicare and Medicaid Services ramping up value-based care programs like chronic care management, it created more demand for vendors who can support this growth. Iowa has a terrific track record of supporting innovative technologies and services in the health care industry, and much of that success is driven by the Iowa Hospital Association. So, it made perfect sense to join this exciting partnership,” said Shane Grivich, co-founder and chief strategy officer, with ChartSpan.
Perry Meyer, executive vice president of IHA and ServiShare, said the partnership aligns with IHA’s values. “We are committed to giving our member hospitals the resources they need to provide high-quality care to their communities. Our partnership with ChartSpan will allow our members to focus on what matters most, their patients. We look forward to this partnership and the benefits it will bring to our members.”
