(Greenville, South Carolina) – September 1, 2020 – ChartSpan, the largest, turnkey chronic care management provider in the U.S., has announced a partnership with Health Initiatives Consulting (HIC), a value-based care consulting firm.

HIC consultants partner with clients that are eager to develop the competencies needed to accelerate the shift to a value-based care delivery model. HIC employs a data-driven approach to align partner resources with their strategic vision. ChartSpan’s Chronic Care Management (CCM) and Annual Wellness Visit services and HIC’s mission are closely aligned. A partnership between ChartSpan and HIC provides the opportunity to extend a partner’s commitment to delivering preventive care beyond their traditional bricks and mortar footprint while improving patient outcomes and reducing cost.

“As we continue to grow our network of partners it is imperative to us that the institutions we target are best of breed and considered leaders in their space. The team at HIC is loaded with talent and experience and we are extremely excited to announce this partnership,” said ChartSpan’s Chief Strategy Officer, Shane Grivich.

“Health Initiatives Consulting (HIC) is very pleased to partner with ChartSpan’s like-minded industry leaders and proven innovators. Their successful track record of maximizing return on investment for Chronic Care Management and Annual Wellness Visits under the CMS initiative is unmatched in the industry and provides an exciting opportunity for HIC to bring new innovation and untapped resources to our clients.
