ChartSpan and Illinois Primary Health Care Association Share Vision for Better Health Outcomes in Illinois

(Greenville, South Carolina) – May 8, 2023 – ChartSpan, a preventative care company, and the Illinois Primary Health Care Association (IPHCA), a nonprofit trade association that serves as the voice of and champion for Illinois’ 53 community health centers, have joined forces to bring value-based care to patients in Illinois. 

ChartSpan collaborates with community health centers across the U.S. to provide patient-first healthcare to Medicare beneficiaries. The company’s solutions include full-service Chronic Care Management, Annual Wellness Visit software, Remote Patient Monitoring enrollment services, and quality improvement services. 

IPHCA is the sole association of community health centers in Illinois. They deliver tools, training, and support to help CHCs—centers that combine medical, dental, behavioral health, pharmacy, and social services—better support their patients. 

If Illinois health centers have patients who need Chronic Care Management, Annual Wellness Visits, or other Medicare services, IPHCA is now able to offer those centers ChartSpan’s programs. In turn, ChartSpan has committed to serving the community health centers of Illinois with consistent, patient-focused care.

Shane Grivich, Co-Founder and Chief Strategy Officer for ChartSpan, expressed his enthusiasm for joining forces with IPHCA. “After beginning work with a key Federally Qualified Health Center in Chicago roughly a year ago, it became evident that our products and services could benefit many other organizations throughout Illinois.
