Medicare's Chronic Care Management (CCM) program is perfectly designed to be a bridgebetween the patient and the care team. While clinics are having to postpone well-check ups and traditional sick visits, a CCM program is an effectiveway to provide vital remote services to patientsand provide an immediate stream of revenue for practices.
To do our part, ChartSpan is offering a RapidResponse™ version ofour remote Chronic Care Management services to any clinic in the United Statesto help address the COVID-19 crisis. ChartSpan will provide the infrastructure for overwhelmed practices tolaunch fast, flexible CCM programs for theMedicare population.

Do I have to sign a contract to get my remote CCM program up and running?
Yes, you will need to sign our Service and Business Associate Agreement, however, the agreement is month to month and you can cancel services for any reason at any time.
How do I get started if our practice has closed or altered its normal clinical workflow and is no longer physically seeing patients?
To qualify for the CCM program as long as you have seen a patient within the last 12 months and they have been diagnosed with 2 or more Chronic Conditions they can qualify for services. Our integration team will remotely check and identify those patients that qualify for the program.
How will training and onboarding with our staff be done?
All training will be done via webinar, email, and recorded videos. No physical interaction will occur at this time. If your office is still remaining open we will mail you some marketing materials for you to use.
How will the patients be enrolled in the program if we aren’t seeing them?
Since 2017 CMS has allowed for verbal consent of the patient for the program. ChartSpan’s consent and enrollment team will conduct this for you as part of your CCM program.
