Terms and Conditions

*All currently enrolled CCM patients in client’s in-house CCM program must transition to ChartSpan. The six-month window begins on the first day of the first full month in which enrollment has begun and ends on the last day of the last full month in which enrollment occurred. ChartSpan must have full access to client’s EHR in order to conduct an accurate and complete Technical Assessment and Client Readiness Assessment, prior to contracting. Client agrees to participate in all ChartSpan CCM enrollment best practices. If ChartSpan fails to increase the size of the customer’s CCM program by 3x, under the terms of this agreement, ChartSpan will refund up to six months of fees for all invoices paid, or provide client an opportunity to credit those fees toward future invoices. Offer is only applied to standard 36 month agreements. ChartSpan reserves the right not to contract with any client.
